The Life Light Health One-on-One Process
I.
Life Light
Health
Inquiry. At
the time a call is returned after an inquiry, the Life Light Health
coordinator arranges an appointment at client's residence or place of
living.
II. Life Light
Health Consultation and Evaluation
During this initial meeting, the needs and requirements of the client,
and the client's general health and condition are identified. The Coordinator then explains
the in-house living support services, and suggests a Care Plan. Most
often, the discussion results in a mutually acceptable Written Care
Plan.
Step 3
Life Light
Health
Written
Care Plan
The Written Care Plan is a document that outlines the specific
services, schedules, client preferences, and special instructions. It
provides common communication to all parties involved in the care of the
client, to best achieve the optimum care that the client needs.
Step 4
Life Light
Health Care Service Agreement
The Life Light Health Agreement is signed only after the client and
other involved parties have received and thoroughly understood the
Client's Rights and Responsibilities including schedules, services and
fees.
Step 5
Life Light
Health Caregiver Selection
Once the Written Care Plan and the Life Light Health Care Service
Agreement are signed, the company does its best to assign the best
caregiver that suits the client’s needs and preferences. We match the
new client with the best CNA/Caregiver, based on personalities, skills,
expertise, availability, and interests.
Step 6
Life Light
Health Caregiver Orientation
Life Light Health provides Caregiver with a copy of the approved Written
Care Plan, directions and other information concerning the client's
care, and report forms that are regularly maintained. Caregivers are not
sent to the client's home until they completely understand the client's
needs, requirements and any special circumstances.
Step 7
Life Light
Health Quality System
Life Light Health's Quality Control Team QCT) makes scheduled and unscheduled
visits to the client's home while the CNA/Caregiver is present to
evaluate the services being provided, the changing needs of the client,
and the client and CNA/Caregiver relationship.
